Kidney Disease Heat Map (uACR and eGFR)

The two tests below measure if you have kidney disease and if you are at risk of kidney disease getting worse.
  • uACR (urine albumin to creatinine ratio) is a urine test. It looks for protein in the urine. When protein is found in the urine, it may mean the kidneys are not working as they should.
  • eGFR (estimated glomerular filtration rate) is a blood test. It looks for waste products in your blood such as your creatinine. It is normal to have a tiny bit of this in your blood. When your kidneys start to not work well, the creatinine begins to increase.  Increased creatinine will lead to a reduction in the eGFR level.

How to Check Your Risk of Chronic Kidney Disease (CKD)

1. Get the results of your eGFR and uACR tests.
2. Place a mark on your eGFR number on the left side of the table.
3. Place a second mark on your uACR number found in the ranges at the top of the column.
4. Draw a line between the dots until they meet together.
The color in the box will tell you about your risk of developing kidney disease.
  • A green result means the no kidney disease or the lowest risk of kidney disease getting worse.
  • A yellow box means you have a risk for your kidney disease to get worse.
  • An orange box means you are at high risk for your kidney disease to get worse.
  • A red box tells you that you are at the highest risk for your kidney disease to get worse.
You will be taught ways to decrease your risk of getting kidney disease or if you have it, ways to prevent your kidney disease from getting worse.
